Purpose

Important:

At the end of this process you must download and PRINT your completed document and instructions, or save them for later printing.
Sign, date and submit the document to the election office address provided to you in the instructions.
IF YOU LIVE IN NEW YORK
Note that even if you fax or email your form, you must send the signed, original, hard copy by mail in order to assure you meet the signature requirements.
IF YOU LIVE IN TEXAS
If you choose to fax your form, a timely faxed FPCA must be followed with a hardcopy mailed FPCA that must be received by the early voting clerk by the fourth business day after the fax.
